POSITION  TITLE:

Self Sufficiency Case Manager –

With Veteran Emphasis

REPORTS TO:  

Youth and Family Services Director

 

 

PURPOSE:

Provide advanced assessment and comprehensive case management service to families requiring assistance with food, clothing, housing, employment, transportation, medical examinations and other services. The services provided are to assists families in working toward self sufficiency.

 

MAJOR RESPONSIBILITIES:

  • Provide extensive interview, assessment, case management and follow up for families requiring financial and other resources assistance, such as personal and family adjustments, finances, employment, food, clothing and housing.
  • Complete intake and assessment on asset building through the Individual Development Accounts (IDA) and Catching A Dream programs.
  • Track account activity for all IDA and Catching A Dream program participants.
  • Provide employment, education and training referrals and other information on career fairs, job readiness resources, and employment and training programs.
  • Refers and connects families with community resources for veterans, Lake County veterans assistance programs, and James A. Lovell Health Care Center.
  • Compiles information and data; and prepares reports for grants and publications.
  • Reviews service plan and performs extensive follow-up to determine quantity and quality of service provided client and status of client’s case.
  • Perform home visits as appropriate.

 

QUALIFICATIONS:

  • Intermediate or advanced computer skills are required
  • Strong written and verbal communication skills
  • Through knowledge of community resources
  • Ability to be self directed
  • Possess good organizational skills and effective time management skills
  • Able to work as a cooperative and supportive team member
  • Have reliable means of transportation
  • Must be able to work flexible hours as needed to accommodate families. This may include hours after regular worked hours.
  • Must possess good client relations skills, as it relates to work environment as well as customer friendly.
  • Must maintain current driver’s license and clean driving record.

 

EDUCATION AND EXPERIENCE REQUIREMENT:

Associates degree in Human Services or related field with three years related experience; or Bachelor degree in Human Services or related field with 2 years related experience; or equivalent years of related experience. Intake, assessment and case management experience required. Experience with veteran case management preferred.
