Self Sufficiency Case Manager –
With Veteran Emphasis
Youth and Family Services Director
Provide advanced assessment and comprehensive case management service to families requiring assistance with food, clothing, housing, employment, transportation, medical examinations and other services. The services provided are to assists families in working toward self sufficiency.

EDUCATION AND EXPERIENCE REQUIREMENT:
Associates degree in Human Services or related field with three years related experience; or Bachelor degree in Human Services or related field with 2 years related experience; or equivalent years of related experience. Intake, assessment and case management experience required. Experience with veteran case management preferred.
